D-SNP: For People Who Have Both Medicare and Medicaid


A Dual Eligible Special Needs Plan is designed for clients enrolled in both Medicare and Medicaid — a group known as "dual eligibles." One of the most common frustrations we hear from these clients is not knowing which program pays for what, or getting caught between two systems that don't seem to talk to each other. A D-SNP coordinates both programs under one plan, reducing confusion and often reducing out-of-pocket costs significantly.

 

D-SNPs are the most common SNP type in the Chicagoland market, with strong plan availability in Cicero, Berwyn, Little Village, and surrounding zip codes. If you have both Medicare and Medicaid in Illinois, a D-SNP is almost certainly worth looking at.

C-SNP: For People Managing a Qualifying Chronic Condition
